Instructions to apply for coverage (for a child-alone under age 19)

1. Contact our Direct Sales Call Center at (800) 358-9727 to get a rate quote or contact your local broker.
2. Complete a separate application for health insurance (Term Life and Accidental Death are not available).
3. Send forms to Golden Rule Insurance Company.

Annual Open Enrollment - July 1 through August 14

1. The first day of the month following receipt of the application, or
2. The date requested.

Enrollment is also allowed throughout the year.
