Under Age 19 Open Enrollment for Kentucky

Instructions to apply for coverage (for a child alone under age 19)

  1. Contact our Direct Sales Call Center at (800) 358-9727 to get a rate quote.
  2. Complete a separate application for health insurance and the Kentucky certification.
  3. Send forms to Golden Rule Insurance Company.

During an open enrollment period Golden Rule must receive the forms between January 1 and January 31.

During a special enrollment period Golden Rule must receive forms within 30 days of the birth, adoption/placement, or termination of prior coverage.
