How to apply for coverage for a child alone under age 19:
- Call us at (800) 358-9727 to get a quote.
- Fill out a health insurance application and the Iowa certification. Term Life and Accidental Death are not available.
- Send forms to Golden Rule Insurance Company.
During open enrollment we must get the forms between July 1 and August 14.
During a special enrollment period we must get forms within 30 days of the qualifying event.
