Under Age 19 Open Enrollment for Colorado

Instructions to apply for coverage (for a child alone under age 19)

  1. Contact our Direct Sales Call Center at (800) 358-9727 to get a rate quote.
  2. Complete a separate application for health insurance and the Colorado certification.
  3. Send forms to Golden Rule Insurance Company.

During an open enrollment period Golden Rule must receive the forms in January or July.

During a special enrollment period due to the applicant’s qualifying event, Golden Rule must receive forms and payment within 30 days of the qualifying event.
