- Complete a separate application for health insurance (Term Life and Accidental Death not available).
- Send forms to Golden Rule Insurance Company.
During an open enrollment period Golden Rule must receive the forms between October 1 and October 31.
During a special enrollment period Golden Rule must receive forms within 30 days of involuntary loss of other insurance for a reason other than fraud, misrepresentation or failure to pay premium (must provide proof of loss).
